The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please Login to our Patient Portal. You will be able to fill out your New Patient forms electronically on that portal. You will not need to print them out, they will be sent to the office once they are completed. If you do not have a New Patient appointment set up, you will need to give the office a call and they will get you on the schedule.

Once you are on the schedule, the Forms, should automatically send to the email you provided. Again, no need to print. Once completed, they will be sent to the office. We do recommend completing the forms as soon as you receive them. This should give our team time to verify your insurance, if you have it, and try to retrieve any dental records from a previous dentist that you may have. Our team will do the best they can to retrieve your previous records as a complimentary service.

We will need your New patient forms done 24 hours before your Appointment time. If you misplaced or did not receive your forms, please give our office a call and they will assist you. Our office phone number is 541-673-7437.

Payment Options

For your convenience, we accept cash, personal checks, debit cards, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
